SAFER: PLL's Trust First Framework in 5 steps

S.A.F.E.R. (short, sticky, department-agnostic)

S — Separate & Safety

Goal: create a safer moment to talk + check immediate danger

  • Get 1:1 time when possible (even 60 seconds)

  • Use a professional interpreter (not family/friends) when needed

  • Quick safety check: “Do you feel safe going back to where you came from today?”

Script: “We do part of every visit/check-in privately with each person.”


A — Ask & Assess

Goal: ask gentle questions + notice indicators (signals, not proof)

  • Ask permission: “Is it okay if I ask a couple safety questions I ask everyone?”

  • Ask clearly, no pressure: “Is anyone hurting you or controlling you?”

  • Assess indicators: fearfulness, controlling companion, inconsistencies, unexplained injuries, frequent visits, self-neglect, withdrawn/flat affect


F — Feelings: Validate

Goal: reduce shame and keep the door open

  • Validate the person (not the details)

  • Don’t interrogate, don’t judge

Scripts:

  • “Thank you for telling me what you could.”

  • “This isn’t your fault.”

  • “You deserve support and options.”


E — Explain Options

Goal: give choices and predictability (this builds trust fast)

  • Explain what you can do today

  • Explain what happens next (and what’s optional vs required)

Script: “Here are your options. You can choose what feels safest today.”


R — Refer & Record

Goal: connect supports + document neutrally

  • Refer: social work, advocate, forensic resources, supervisor, mandated reporting pathway (if relevant)

  • Record: neutral language, patient’s words when possible, avoid blame/shame phrasing

Script: “My job is to support your safety and health—and connect you to help.”
